Metabolic age calculator

Six questions, one tape measure. How old your metabolism behaves — which is often not the number on your birth certificate — built around waist-to-height rather than BMI.

01The calculator

Measure your waist before you answer

At the navel, first thing in the morning, tape snug but not compressing. That single number carries most of the weight in this calculation, because waist-to-height tracks visceral fat far better than BMI tracks anything.

  • No email, no account, nothing stored on a server
  • Waist-to-height as the primary input, not scale weight
  • Sleep and activity adjust around it, honestly weighted
  • You can change your last answer at any point

“Metabolic age” has no standardised clinical definition. This is a heuristic built on the inputs with the best supporting evidence — useful for spotting which variables are ageing your metabolism faster than the calendar, and useless as a diagnosis.

Metabolic Age

Question 1 of 6

I'm from the HealthCalculus panel. I'll ask you six quick things — no forms, just answer as you would in a consult.

02What drives it

Four levers, in the order they matter

Bar length is our weighting of how much each input moves the estimate, not a measured effect size. The order is what you should act on.

Waist relative to height

Lever 1

Waist-to-height tracks visceral fat, and visceral fat is what actually drives insulin resistance — which is why it outperforms BMI as a risk marker. It is also the fastest-moving variable on this list, which makes it the one worth measuring first.

Resistance training and lean mass

Lever 2

Muscle is metabolic infrastructure, not decoration. Resistance training improves insulin sensitivity independently of weight change, and after fifty, lean mass needs actively defending rather than passively keeping.

Sleep duration and timing

Lever 3

Short, misaligned sleep produces prediabetic-grade insulin resistance in healthy adults within weeks, and days of restriction are enough to disrupt appetite hormones. It is the cheapest lever here and the most commonly ignored.

Dietary pattern, especially ultra-processed intake

Lever 4

In a controlled inpatient trial, an ad-libitum ultra-processed menu drove roughly 500 kcal a day more intake than a matched minimally-processed one. Large reviews link the same pattern to cardiometabolic outcomes across dozens of endpoints.

03Beyond the heuristic

If you want a measured biological age

These produce an actual measured value rather than an estimate from habits. Worth considering if you are actively optimising or want a real baseline to track against.

Cost of a blood panel

Levine PhenoAge

Calculated from nine routine bloodwork values — albumin, creatinine, glucose, CRP, lymphocyte percentage, MCV, RDW, alkaline phosphatase, white cell count — plus chronological age. Free calculators exist; you only need the panel.

Mid-range

GlycanAge

Measures glycan profiles in blood, validated against inflammation and metabolic-syndrome outcomes. Sits between a heuristic and a methylation clock in both cost and rigour.

$200–500

Methylation clocks

DNA-methylation based biological-age tests in the Horvath lineage, sold direct to consumers. The most rigorous biological-age proxy currently available outside a research setting.

We do not sell any of these

No affiliate relationship exists with any biological-age testing company, and nothing on this page is paid placement. If you only ever run one panel, the nine-value bloodwork behind PhenoAge is the cheapest route to real numbers — and your physician can order most of it as routine.

04What to do with it

A number you re-measure beats a number you screenshot

Measure the same way, change one input, come back in eight weeks. Direction of travel is the only part of this estimate that is genuinely reliable.

Measure

Navel, morning, snug tape. Write it down with the date.

Pick one input

Sleep or training, usually — not a whole new diet.

Hold it eight weeks

Long enough for waist to move measurably.

Re-measure

Same conditions. Compare the trend, not the number.

About this number

It has no standardised clinical definition, and any tool claiming otherwise is overstating things. Different scales and apps use different formulas. What they share is an attempt to express how far your measurable metabolic profile sits from what is typical for someone your chronological age.

Ours is built around waist-to-height ratio, with activity and sleep adjusting around it, because those are the inputs with the most consistent supporting evidence and the ones you can actually measure at home. The output is a directional estimate, not a biomarker.

Get a profile, not just a number

The assessment looks at the whole picture — markers, history, what you have already tried — and points you at the protocol the research supports for your profile rather than the one that is trending.

Educational only, not medical advice. “Metabolic age” is a heuristic without a standardised clinical definition; this calculator produces an estimate, not a diagnosis.
